Common questions

Do I really need TRT or am I just getting older?

Honest answer: both can be true. Testosterone declines naturally with age, but for some men the decline is steeper or more clinically significant. Symptoms alone aren't enough — we need labs. If your testosterone is genuinely low AND you have symptoms AND we've ruled out other causes (sleep apnea, depression, thyroid issues, medications), TRT may be appropriate. If your testosterone is borderline, we'll talk through the tradeoffs.

What's the best delivery method?

Depends on your life. Injections (weekly) give steady levels and are the most cost-effective, but require you to inject yourself or come in regularly. BioTE pellets are 'set it and forget it' for 4–6 months but cost more. Creams are easiest to start and stop but require daily application and careful handling to avoid transferring to family members. We help you choose.

Will TRT affect my fertility?

Yes — this is the most important conversation to have BEFORE starting. TRT suppresses your body's natural testosterone production AND can suppress sperm production. For men who want to preserve fertility, we discuss alternatives (HCG, clomiphene) or add medications to TRT that preserve fertility. Don't start TRT without having this conversation.

What are the side effects?

Common: acne, increased hematocrit (red blood cell count), mild fluid retention, changes in libido (usually increase). Less common: breast tissue tenderness, mood changes, sleep apnea worsening. We monitor labs to catch issues early and adjust dosing as needed.

Will TRT cause prostate cancer?

Current evidence suggests TRT does NOT cause prostate cancer in men without pre-existing disease. However, TRT can potentially accelerate growth of existing prostate cancer. That's why we monitor PSA at baseline and during therapy. Men with active prostate cancer should not be on TRT.

How long will I need to be on TRT?

Most men who start TRT stay on it long-term because their body's natural production has been suppressed and won't fully recover. This is a real consideration that needs honest discussion. If you want to stop someday, there are protocols to support recovery of natural function, but they don't always succeed completely.

Will my insurance cover TRT?

If your testosterone is clinically low and your provider documents medical necessity, some insurance plans will cover TRT. We don't bill insurance directly — we provide cash-pay programs with HSA/FSA-compatible receipts, and we can give you documentation to submit to your insurance for partial reimbursement if applicable.
